Please find the following forms available on the State Court Clerk's webpage. Please note that some of the forms are interactive:
- Change of Address Form (PDF)
- It is a defendant's responsibility to notify the court in writing of any change of address. Sureties and attorneys are also responsible for notifying the court for any change of address.
- The change of address must be submitted in each case in which the defendant, surety, and/or attorney is associated.
- If this is not done, it could result in not receiving notification of a court date change and this could result in a bench warrant being issued for failure to appear.
- Ignition Interlock Waiver (PDF)
- Please read the Ignition Interlock Waiver Standing Order before you print the Ignition Interlock Waiver request.
- Notice: Pursuant to O.C.G.A. §42-8-111(b), “If a court grants an exemption from the ignition interlock device requirements, such person shall not be eligible for a limited driving permit or any other driving privilege for a period of one year.”
